Position Summary

Department: WellSpring Community-Based Team

Supervisor: Community-Based Services Director, Clinical Supervisor 

GOAL:  To advance the mission of WellSpring Counseling, Central Ohio Youth for Christ, and subsidiaries by providing high quality community-based services to clients, their families/support teams. 

KEY OBJECTIVES

  • Provide clinical community-based services
  • Maintain all compliance standards and licensure requirements
  • Fulfill staff administrative responsibilities
  • Fulfill COYFC spiritual responsibilities

SPECIFIC DUTIES 

Responsibilities include, but are not limited to, the following: 

Community-Based Services:  

  • Provide individual and family based clinical services to youth and families who are at risk and are active clients engaged in WellSpring Community-Based Services.
  • Complete a Comprehensive Diagnostic Assessment and/or Update for all new clinical services clients upon intake and update annually. 
  • Provide level of care and treatment recommendations based on the Diagnostic Assessment and medical necessity.
  • Review, and assist with the ongoing development and implementation of the clients individualized treatment plans (ITPs).
  • Collaboration with the clinical services team with implementing a trauma informed continuum of care and clinical services throughout Gracehaven and WellSpring. 
  • Assist Clinical Supervisor with reviewing and screening referrals, as well as onboarding and intake of new clients

Compliance Responsibilities 

  • Maintain an active license that is in good standing with the state of Ohio Social Work and Counselors Board.
  • Participates in professional development (e.g. training, workshops, conferences, etc.) to maintain professional knowledge and proficiency, as well as the Continuing Education Units (CEUs) necessary to maintain (or progress toward) appropriate licensure and credentialing.  
  • Comply with all WellSpring policies and procedures, as well as all current federal, state, and local standards, guidelines, and regulations governing WellSpring Therapeutic Services, including but not limited to ensuring that the confidentiality of the client is maintained in accordance with federal HIPAA regulations.
  • Maintains accurate and timely documentation/outcomes data/records and service delivery requirements, as set forth by the agency policy & procedures and OhioMHAS.
  • Meets monthly productivity standards for the position. 
  • Participates in program quality improvement activities and maintains quality assurance standards.
  • Monitor implementation, revision of the ITP plan, meeting criteria for medical necessity of this plan with the interdisciplinary team according to regulatory guidelines 

Administrative Staff Responsibilities 

  • Provide accurate and timely documentation of service delivery and client’s progress, in accordance with the treatment goals and within the timelines set forth by the agency and regulatory guidelines.
  • Attends COYFC/WellSpring All Staff training and staff meetings. 
  • Actively participate in supervision, community team meetings, case reviews, and other intra-agency meetings and training opportunities for the role. 
  • Connect clients and their families to faith-based community resources and ministries, including YFC if applicable 
  • Work in conjunction with WellSpring Community-Based Clinical Director to identify and implement an evidence based treatment model for Community Based Treatment Services.
  • Performs all other duties as assigned by the Community-Based Clinical Director and/or Community-Based Clinical Supervisor.

  • Explain COYFC faith values and beliefs to clients and families, if asked
